N2  - This study presents the development of an information  system tor a medical clinic of an academic institution . The  existing Medical Record System of the medical clinic is studied and a new system is designed . This Medical Record system store the medical history  information as well as the current medical examination  records of staff and students of AlT . In particular, the  psychiatric test results of student are also stored . The  system generates monthly and annual report. for management  purposes. certain inquires on individual medication and  treatment are also provided. The techniques of structured Design and Hierarchy plus  Input-Process - Output (HIPO) are used throughout the design .  The new Medical Record System is designed to run under IBM  VM/370 CMS system using VSAM/KSDS file structure .
