520 _a Access to public services for TB treatment was hindered by financial, socio-cultural, health system and geographical barriers in the rural Balkh district of Afghanistan. The objective of this study was to explore and evaluate the institutional arrangements, methods and process of delivery of public service for TB treatment, analyze the access to public services for TB treatment by gender and socio-economic condition of rural people, their seeking and securing behavior for TB treatment and evaluate the satisfaction with the process, methods and quality of public services provided. The study was carried out in the rural areas of Balkh district in Afghanistan. An exploratory cum evaluation study was used and applied a questionnaire and case study to gather the primary qualitative and quantitative data while the secondary data was used to analyze the policy statement. The adult participants were identified from the TB register at the health center for both the male and the female. It was found out in this study that cultural beliefs, transport, work lord, low income hindered access for both male and female while the female faced social marginalization in the event of being recognized as a TB patient than the male. The female patients expressed concern on the level of satisfaction as their privacy was threatened by being in the same waiting room with the male TB patients counterparts. The institutional arrangements for TB treatment were integrated in the primary health care and implemented through contracted NGOs to run the programme to reach the rural poor but because of lack of coordination among stakeholders, lack of staff, lack of financial resources, logistic problems and insecurity from the NGOs and lack of monitoring and supervision resulted in the TB programme implementation failing to reach the rural areas. The process of TB treatment was found out to be complex and long to adhere to and the DOTs method of treatment required high levels of compliance.
